Persons and family members who have sustained a hip fracture or cared for someone who has sustained a hip fracture are being invited to participate in an online survey related to their rehabilitative care needs. Please share this opportunity with your patients and their families in your Local Health Integration Network (LHIN) or hospital.
The results from this survey will support the work that the Rehabilitative Care Alliance (RCA) is undertaking on the co-design of a hip fracture capacity plan. The survey is intended for persons who have had a hip fracture and their families to share their rehabilitative care needs after a hip fracture.
While it will be important for us to get participation from people who have had a hip fracture, we are also seeking participation from family members who have an understanding of the rehabilitative care needs of people following hip fracture. Additional details are provided in the following 2 documents:
- Hip Fracture Person/Family Member Invitation: Letter of Information
- Hip Fracture Person/Family Member Needs: Information and Consent
