New guide helps OHTs address rehab services

The Rehabilitative Care Alliance (RCA) and GTA Rehab Network have developed a brief guide to support providers developing Ontario Health Team (OHT) applications.

Rehabilitative care is an essential element of Ontario’s new vision for connected care. Evidence shows that rehabilitative care improves health outcomes for patients, shortens hospital stays and helps older adults to maintain function and avoid hospitalization.

Rehabilitative Care: An Essential Component of Connected Care provides an overview of how rehabilitative care services across the care continuum help patients regain health and quality of life and can help OHT’s deliver on the quadruple aim of better patient, caregiver and provider experience, better health outcomes and better value and efficiency.

The guide also provides practical suggestions for OHT applicants.
