Hospitals, CCACs, and community clinics from across the LHINs are sharing information and strategies as they implement the new definitions frameworks.
The conversations are occurring with the help of the RCA's newly formed Health Service Providers Definitions Implementation Group (HSP DIG). HSP DIG was established as a forum to allow frontline organizations to learn from each other as they implement the two frameworks. The group, which has more than 60 members, met for the first time in September and includes directors and managers responsible for rolling out the new definitions frameworks in their organizations.
For the people involved in the "nitty gritty" of implementation, the group provides an opportunity to talk with others about challenges and to learn how others are implementing the guidelines in their organizations.
As with any initiative where there is a shift from current state to a future state model, the "devil is in the details." The discussion at the first meeting indicated that organizations are actively engaged in implementing the Rehabilitative Care Frameworks and benefited from the discussion with other health service providers.
The group, which will meet periodically, will also identify any cross-LHIN issues that require a provincial approach. These will be addressed by the Definitions and Capacity Planning Group who is leading this initiative for the RCA.
All LHINs will be reporting on their implementation progress in December.
