A new white paper from the RCA provides health system decision-makers with recommendations to improve the spread of innovative and cost-effective community-based models of rehabilitative care.

Community-Based Rehabilitation: Providing High-Value Care in the Community is a four-part white paper that explores the critical role that community-based rehabilitation plays in the Ontario health system.

  • Part 1The Value of Community-Based Rehabilitation: An Overview introduces community-based rehabilitation and provides an overview of its value.
  • Part 2In-Home Rehabilitation draws on evidence to describe four categories of in-home rehabilitative care based on patient needs—prevention, post injury/illness, progressive/chronic and maintenance. Key best practices and examples of effective models of rehabilitative care and their outcomes are provided for each category.

    Part 2 also provides recommendations on the optimal organization and provision of in-home rehabilitative care to improve patient outcomes and reduce cost, and discusses pandemic considerations, including the use of virtual care.

  • Parts 3 and 4 (Ambulatory-Based Rehabilitation and Rehabilitative Care in Primary Care) will be released in 2021.
