New group to focus on improving transitions

The RCA is recruiting members for a new expert group focused on improving transitions. The initiative reflects the RCA’s work in system evaluation and its commitment to using performance data to support quality improvement initiatives.
Provincial stakeholders and subject matter experts recently identified a priority area for improvement: improving transitions from inpatient hospital care to in-home rehabilitative care. The RCA would like to hear from individuals interested in participating in a time-limited (August 2021 – February 2022) subject matter expert group to look at this issue. Specifically, we are seeking representatives from across the province who have first-hand experience with transitions from an acute care hospital to in-home rehab and, ideally, who have worked within their networks to improve those transitions.
