Facilitated implementation series supports rehab care for older adults

The Rehabilitative Care Alliance and Provincial Geriatrics Leadership Ontario have launched a Facilitating Implementation Conversations series with regions (Sault Ste Marie, Thunder Bay and Ottawa) interested in adopting the Rehabilitative Care for Older Adults Living With/At Risk of Frailty Framework.
Early adopter regions are providing input to improve the series, which offers a joint rehabilitation/specialized geriatrics process to support organizations in ensuring best practice rehabilitative care for older adults as outlined in the new framework. Participating teams receive facilitator support and guidance to:
- scope problems and systematically frame implementation issues to identify those most relevant;
- enhance local relationships by ensuring connections between support-seeking organizations/teams, regional rehabilitative leads and specialized geriatric services;
- build and execute local implementation plans related to rehabilitative care for older adults; and
- identify relevant planning and implementation supports and resources.
Participants are working through defined steps to study their population demand, define current capacity, maximize existing capacity and assess their readiness for future investments. Their collective work will be showcased in fall 2023.
