In progress: paramedic post-fall pathway

The RCA is working with paramedic services and local specialized geriatric services in numerous regions to adapt and implement its post-fall pathway for use by community paramedics. As they are often called to assist people who have fallen and not been transported to hospital, paramedics are ideally positioned in the patient journey to refer these individuals to appropriate geriatric rehabilitative care services to help prevent future falls. A comprehensive orientation will also help build the geriatric capacity of community paramedics.
Over the coming months, communities throughout Ontario will prepare for and begin to implement the Paramedic Post-Fall Pathway. A quality improvement approach, including data collection and analysis of patient journeys and experiential learnings, will be used to develop a more comprehensive implementation plan for spread and scale across Ontario.
