Rehab Transitions Self-Assessment Tool

The Rehab Transitions Self-Assessment Tool was designed by the Rehab Transitions Quality Improvement Working Group to support clinicians, practice leads, managers and decision-makers in conveying essential patient information to in-home rehab teams when a patient transitions home from hospital. It is intended to serve as a self-assessment tool that can be referenced by individuals, organizations or Ontario Health Teams to promote the transfer of pertinent information that is timely and relevant to support the assessment and treatment of patients at the first in-home rehab visit.
The tool has been implemented in the East region in partnership with Kingston Health Sciences Centre, KHSC@Home, Kaymar Rehabilitation and Home and Community Care Support Services Southeast. Implementation data has now been analyzed and will be shared with participating partners this month.
