Post-fall pathway implementation

The RCA continues to work with Ontario Health Teams across the province to support the implementation of post-fall pathways.
As part of a commitment to continuous quality improvement, the RCA updates both pathways and related resources and toolkits as new learnings arise through the implementation process.
