Updates to the RCA’s directory of community clinics and hospital outpatient programs accepting external physiotherapy referrals for orthopedic bundled care patients are nearly complete. The centralized contact list was first created to help facilitate smooth referrals and streamline transitions across the continuum of care, regardless of where a patient resides in the province.

Thank you to all partners for providing updated information to strengthen the list further. When fully updated, it will be posted on the Bundled Care Community of Practice Quorum and shared with the RCA’s Bundled Care Task Group.
