The RCA is updating its Expanded Direct Access Priority Process (Expanded DAPP). The original DAPP process was first developed in 2015 to support timely access to rehabilitative care for older adults. It supported health care providers in identifying individuals with restorative potential who required inpatient and/or community-based rehabilitative care and facilitated referrals directly from the community or emergency department, providing a direct pathway to inpatient rehab and removing acute care as the sole point of access.

Now renamed the Rehab Access Pathway (RAP), the process is being refreshed in collaboration with subject matter experts to ensure it reflects current best practices. Once live, the RAP will be available for piloting with interested organizations. A quality improvement approach, similar to the RCA’s ongoing post-fall pathway implementation, is envisioned as a means of offering support and sharing lessons to ensure successful adoption.
