Post-Fall Pathways Gain Momentum Across Ontario

In April, the RCA released a special-edition newsletter highlighting how partners across Ontario are advancing pathways to rehab for older adults with frailty following a fall. The publication showcases implementation efforts from across the province and highlights how organizations are adapting and integrating post-fall pathways within their local health care systems.
The communique features examples from community paramedicine, primary care, and emergency department settings, demonstrating how regions are working to improve coordination, strengthen connections to rehabilitation services, and support timely access to care for older adults following a fall. It also highlights lessons learned through implementation and identifies opportunities to support future spread and scale.
