The RCA has released a special-edition newsletter showcasing how partners across Ontario are advancing pathways to rehabilitative care for older adults with frailty following a fall.

This work responds to a well-established challenge: falls often mark a turning point for older adults, contributing to functional decline, repeated emergency department visits, and increasing care needs. Strengthening access to timely, coordinated rehabilitation is critical to supporting recovery and maintaining independence.

Across the province, regions are implementing and adapting the RCA’s post-fall pathways, which connect patients more effectively to care. These efforts span community paramedicine, primary care, and emergency departments, with a shared focus on earlier identification, improved coordination, and more consistent access to rehabilitation services.

What is emerging is a more proactive and connected approach. In many cases, partners are achieving meaningful improvements by better aligning existing resources rather than introducing new ones.

The newsletter highlights examples from across Ontario, including early adoption in Northern communities, regional coordination in the Greater Toronto Area, and locally driven innovations in Central, East, and West regions. Together, these efforts reflect growing momentum and a shared commitment to improving post-fall care.

The RCA continues to work with partners to support further implementation and spread of these pathways across the province.
