Online survey for patients post hip fracture

Persons and family members who have sustained a hip fracture or cared for someone who has sustained a hip fracture are being invited to participate in an online survey related to their rehabilitative care needs. Please share this opportunity with your patients and their families in your Local Health Integration Network (LHIN) or hospital.
The results from this survey will support the work that the Rehabilitative Care Alliance (RCA) is undertaking on the co-design of a hip fracture capacity plan. The survey is intended for persons who have had a hip fracture and their families to share their rehabilitative care needs after a hip fracture.
While it will be important for us to get participation from people who have had a hip fracture, we are also seeking participation from family members who have an understanding of the rehabilitative care needs of people following hip fracture. Kim Young is the principal investigator for this project. Kim also serves as the Advisor, Planning, Hamilton Niagara Haldimand Brant (HNHB) Local Health Integration Network (LHIN). In her role at the HNHB LHIN, she is the Rehabilitative Care Alliance (RCA) lead for the HNHB LHIN and is a member of the provincial RCA hip fracture subject matter expert group as well as the patient and family focus group. She is also a member of the provincial RCA capacity planning advisory committee and task group.
